A/V Repair Request Form
Small repairs and installations under $5,000 can be requested via an On-Campus Requisition form. Please download the form, fill out the necessary information including a detailed description, and the building/room where work is to be completed. If your unsure what to ask for, simply call our help line, 472-5511 for assistance.
